To Report a Lost or Stolen Passport |
As of January 1, 2005 the U.S. Passport Office requires both photos to be signed on the back by the applicant.
The following instructions apply to you if you have lost or someone has stolen your passport. In addition you must have been at least 16 years old with the passport was issued. If these requirements do not apply to you, please contact our office for further instructions.
- Completed pink application form, DSP-82
- Completed DS-64 Statement Regarding Lost or Stolen Passport
- 2 passport photographs. (2x2) taken within the last 3 months
- Most recently issued passport
- Make your check or money order payable to the "U.S. State Department" for $75.00
- Proof of departure:
- Copy of your airline tickets of flight itinerary
- Letter from you company stating:
- Full name of applicant
- Correct date of departure
- Destination
- Purpose of trip
- A signed release letter authorizing "IRATEX Passport & Visa or, "Passport & Visa
Expeditors, to collect the passport on your behalf. The release letter must be signed and dated by the applicant.

To avoid any delays in processing your Passport please call our offices before sending us your documents. This will insure a complete and accurate packet.
