EXPEDITING AMENDMENTS FOR US PASSPORTS
The Amendment is used  for adding pages to your current passport.

REQUIREMENTS

1)    One DS-4085 form, completed and signed. (See below)

2)    Proof of departure. Either a copy of your flight itinerary or ticket or a letter on company letterhead stating the applicant's name, date of intended departure and destination

3)    Release/Authorization form giving your written permission to the State Department authorizing the release of your passport to us. (See below)
